When you or a loved one is faced with a terminal illness, you need expertise you can trust. Great Lakes Hospice has the experience and qualifications you want and deserve during this difficult time. Great Lakes Hospice offers the complete range of care required by terminally ill patients and their families. Our team of professionals and volunteers include:

We provide access to all levels of care on an as-needed basis. Medical supplies and equipment, medications for symptom management and other services are also available through our program. We bring care to you, allowing you to be where you are most comfortable.

A Plan for Living

The hospice philosophy promotes a plan for living that provides you with measures to improve the quality of life. The care you receive will be coordinated between those whom you know and trust and our trained and compassionate staff. Eligibility requirements include:

  • Licensed physician authorizes hospice care and agrees to participate in planning of care.
  • Support person(s) are identified and agree to participate in planning of care.
  • Patient has a life expectancy of six months or less and seeks comfort measures instead of aggressive treatment.
  • Patient desires hospice service and is aware of diagnosis and prognosis.
  • Care can be provided in the home, assisted living facilities or contracted extended care facilities.
  • Patient's home is adequate for safe and effective care.
  • Patient lives within Erie County.

Volunteers

Volunteers are an important component to hospice care. They provide support and comfort for the patient and family members. Bereavement

Great Lakes Hospice provides a number of different programs for the grieving family including:

  • Bereavement visits - Our staff will make a bereavement visit with the families they have worked with upon request and need.
  • Phone calls - We have a follow-up phone call program which calls the caregivers on a periodic basis for at least 13 months.
  • Bereavement mail program - Our mail program offers support and information with four mailings containing information on the grieving process, as well as resources available.
  • Bereavement Support Groups – held the first and third Thursday of the month from 5:30 to 7:00 p.m. at 1700 Peach Street, Suite 244. To learn more call 814-877-6120.
  • Bereavement counseling - Great Lakes Hospice has bereavement counseling available for individuals and families. We also offer referrals to other community resources which might fit individual or family needs.
  • Memorial service: Services in the Spring and Fall to remember patients who have died during the previous months.

Memorials

Many family members and friends like to recognize their loved-ones by offering a memorial donation to Great Lakes Hospice. Our memorial funds are managed through the Hamot Second Century Foundation. We use these donations throughout the year to provide education and support to patients and family members within our hospice program. The support of the community to these efforts is greatly appreciated. Memorials can be sent in care of your loved one to:
